Child Health Report Update

Child Health Report must be completed and signed by your child's physician and returned to Sue to be placed in your child's file.

Children under 2 years must have a bi-annual health report update; children over 2 years must have an annual health report update on file.

Immunization print-out from your child's physician must be submitted along with the Child Health Report.
